Please complete our Application for Admission. Include: current report cards and/or transcripts, immunization records, IEP/504 plans (if applicable), and a birth certificate.
 
 
 
 
 
Academic Assessment
 
 
Academic Assessment & Interview: Application Fee & Documentation
We will contact you to schedule an academic assessment and an interview with the school level Principal. The academic assessment and interview will allow for the opportunity to best shape your child's plan for success, along with making your transition as smooth as possible. After August 1, a $1000 per child deposit is required to be paid at the time of the Principal interview

Complete an Enrollment Packet
You will receive a welcome email inviting you to set up your FACTS Family Portal. Please complete the Enrollment Packet, which includes providing medical information, designating emergency contacts, selecting your tuition plan, and uploading any documents missed during the application, including court orders, if applicable.

Your approval is confirmed once the Enrollment Packet in your FACTS Family Portal is complete and your one month tuition deposit has been processed.

Your official letter of acceptance will arrive via email outlining important dates and details, including your school start date.

Things to Know
Placement testing for all applicants in grades K-12 will be scheduled by Life Christian Academy. An interview with the school level principal will be scheduled for applicants after testing has been completed and all required forms have been received.

While we do not guarantee placement for siblings, we do give preference to siblings when a sibling is already enrolled.

Enrollment Documents Required By Life Christian Academy 

  •  Birth Certificate
  •  Immunization records
  •  Reference letters (two for grades 6-12)
  •  Copies of previous year and current year report cards
  •  Copies of standardized testing results for the last two years
  •  Copy of high school transcript (grades 9-12)
  •  Copies of court documents, custody paperwork, and/or Parenting Plan
